We Accept Insurance

The following listing provides general information regarding insurance providers that we have contracts with. Your actual benefits and coverages will be determined by your insurance provider. As always, we suggest contacting your plan administrator to verify your specific chiropractic benefits, pre-notification or referral requirements as not all charges are always covered.

In-Network or Out-of-Network, we will gladly process and file your insurance claims for you. When booking your appointment online or in person, make sure you have your insurance card available so we can attempt to verify your available benefits and potential discounts for care in our clinic and send you a good faith estimate of what to expect. VETERANS: (Veteran's Choice Program or Fee Basis) - REFERRAL REQUIRED

We proudly work with TriWest as part of the Veteran’s Choice Program and VA Fee-Basis for care at little to no cost to you! Simply, create a request to your VA PCP by appointment, your veteran portal or call the phone number on your TriWest card and request your authorization for chiropractic care. Once approved, you’ll be able to choose us as your chiropractic provider of choice and we will call you as soon as we receive your authorization for care. Medicare (non-participating)
Medicare

We are a non-participating provider with Medicare. Medicare members can still seek care through our office but will be required to pay at the time of service. Your claim will still be filed, and if reimbursement is eligible Medicare may reimburse you directly for a portion of your bill.  Medicare provides Chiropractic benefits for Spinal Manipulation (Spinal Adjustment) ONLY. Other services such as Exam, Manual Therapies, Therapeutic Procedures or dry needling may be beneficial and clinically indicated or necessary for care – these charges are available as a self-pay option or may be eligible for reimbursement by a secondary insurance provider. Worker's Comp - PRE-AUTHORIZATION REQUIRED

Work-Related injuries need to be reported to you employer. Your employer is responsible for providing you with a panel of physicians you may see for your work-related injury. If your employer gives you the option to choose your own chiropractor, we are happy to work with them for your worker’s comp case. We require a signed employer’s authorization before accepting a work-related injury case.

Motor Vehicle Accident / Auto Coverage

We are not currently accepting Motor Vehicle Accident cases at this time.

No Insurance

No insurance? No problem! Estimates of care is based on case complexity or services performed. Please call our office at 615-537-5520 for more information.

Don’t see your insurance provider listed? You may have a smaller insurance company that contracts through one of the larger providers listed above. Please contact your plan administrator for details or call our office with your Subscriber ID #, Patient Name, Patient Date of Birth and Customer Service/Provider Service telephone number. We will gladly check your benefits and see what discounts you may be eligible for.
