Become a Patient
Becoming an Active Spine and Joint Center Patient is the first step in taking control of your health and living a life with minimal or no pain. It’s as simple as calling our friendly office staff or you can request an appointment online using the following button.
Please arrive 10 – 15 minutes prior to your scheduled appointment time to complete any other registration forms and to allow time to verify your insurance benefits (if applicable). Your prompt arrival helps us make sure we have any other additional forms required by insurance completed and helps our physicians stay on schedule.
How Long will my Appointment Take?
Allow 60 – 90 minutes for your first visit for our team to complete a thorough comprehensive exam on your initial visit and at any new injury examination.
What Should I Wear?
Dress in comfortable clothing, such as shorts and t-shirt or workout clothing that allows free movement. You may also bring a change of clothing with you if preferred.
What Do I Need to Bring With Me?
Please bring any applicable imaging reports you have for recent X-Ray, MRI or CT scans. If you do not have a copy of the imaging report, a Medical Records Release form is available for you to download and send to the clinic that performed the procedure (see link to the right) so we may have these at the time or prior to your appointment time.
A PHOTO ID and VALID INSURANCE card if you wish to bill insurance.
A copy of your ACTIVE MEDICATIONS.
How Soon Can I Be Seen?
We will always work to get you seen by a physician as soon as possible, same day if necessary. Appointments requested at peak times (7am – 10am or 4pm – 6pm) may have a waiting period as these are our most popular appointment times. It’s important to us for the physician to have the time available to complete a thorough evaluation of your case.
